The 5MHz Consolidated log needs to be completed exactly to the following required format: 

Column heading  Description & Format
Day Day number (leading zeros not required)
Month Month number (leading zeros not required)
Year The year in its 4 digit format.
HourGMT 24 hour representation of time in GMT, leading zeros not required.
Minute Minute number, leading zeros not required
StnReporting Callsign of station reporting (with no gaps)
StnRepQTH 6 character QTH Locator (or SOTA Ref number) of station reportiing, with no gaps and in upper case.  No other information please. 
StnHrdorWkd Callsign of station worked or heard (with no gaps)
StnHrdorWkdQTH 6 character QTH Locator (or SOTA Ref number) of station heard or worked, with no gaps and in upper case.  No other information please. 
SentS "Sent" part of SINPO - reception report in case of reception report, see Assessment of SINPO.  
SentI "Sent" part of SINPO - reception report in case of reception report, see Assessment of SINPO
SentN "Sent" part of SINPO - reception report in case of reception report, see Assessment of SINPO
SentP "Sent" part of SINPO - reception report in case of reception report, see Assessment of SINPO

Steps Column only used for reporting the 5MHz beacons.  The number of steps heard is entered into this column.
BandwidthHz This column is reserved for users submitting reports using very narrowband processing teqchniques (other than the automated gb3ral.exe, see 5MHz automatic beacon monitoring).  For most reporters this column should be left blank
RecS "Received" part of SINPO - if no report received or the normal RS or RST is sent, please leave column blank
RecI  "Received" part of SINPO - if no report received or the normal RS or RST is sent, please leave column blank
RecN "Received" part of SINPO - if no report received or the normal RS or RST is sent, please leave column blank
RecP "Received" part of SINPO - if no report received or the normal RS or RST is sent, please leave column blank
RecO "Received" part of SINPO - if no report received or the normal RS or RST is sent, please leave column blank
Channel Insert channel designator - FA, FB, FC, FE, FK, FLor FM. 
Mode Please use mode designators SSB, CW, DRM, PSK, PSK31, FS-1049, FS-1052, SSTV and MT63 - please don't use lowercase or other terms like USB, J3E, A1A, etc.
PowerW Transmit power in watts, rounded up to the nearest integer.  For reception reports leave blank.
AerialType Aerial type.  Please only use one of the following codes: RD for resonant dipole, NRD for non-resonant dipole, V for vertical, L for small loop and O for other.
HeightAGL Aerial height above ground level in metres
AerialPolarisation This is for the predominant polarisation of the aerial - H for horizontal, V for vertical and M for mixed
AerialAlignment Please use the appropriate orientation such as N/S, NNE/SSW, NE/SW, E/W, ESE/WNW, SE/NW, SSE/NNW or O for omni-directional
DataSource Historically this field is left blank for logs compiled using EXCEL.  Currently, for logs that are generated by the 5MHzLog program a checksum is generated automatically.  This is changed during the pre-uploading validation to "5MHzLog", which is what is seen against such records in the Log Table in the database."
